This form is for cosmetic adaptations of the original Power and Control Wheel. This means that you will keep the wording and the content exactly as it is, but you would like to add color to the wheel or change the font or something else that would not change the meaning or intent of the wheel.
We request that these adaptations not be published to the web.
There is a $25 service fee associated with these requests.
We will need to review certain information for your request. Please have the following ready before you continue with your form:
- Explanation for why you want to create this adaptation
- Copies of the materials that will include the wheel
- Copies of the wheel image with the requested adaptations
