There is also a $25 service fee associated with these requests.
We will need to review certain materials for your request. Please have the following available before you begin your form:
- Title of the publication/curriculum/thesis/article
- Name of publishing company and contact information, if applicable
- Abstract giving a general overview of the publication
- Reasons you want to include the Power and Control Wheel in your publication
- Copies of the exact wheels you would like to use
- Copies of the chapter or section in which you would like to include the wheel
- Credit card
